What is Home Care?
At Sweetwater Care we provider a wide array of services intended to help older people live as comfortably and independently as possible. Examples range from basic transportation, cooking, or cleaning to complex medical care. Our services are meant to help older people live comfortably and independently.
Attendant Care (ATC)
This service provides assistance to allow a member to remain in their home and participate in community activities by attaining or maintaining personal cleanliness, activities of daily living, and safe and sanitary living conditions. ATC may include the following as determined by the member's assessed needs:

Meal Prep: Meal preparation and clean up (e.g., meal planning, preparing foods, special diets, clean-up, and storing foods). ​This also includes eating and assistance with eating

Washroom Prep: Bathing (e.g., washing, drying, transferring, adjusting water, and setting up equipment)

Dressing Assistance: Dressing and grooming (e.g., selecting clothes, taking off and putting on clothes, fastening braces and splints, oral hygiene, nail care, shaving, and hairstyling)

Mobility Assistance: Mobility (e.g., physical guidance or assisting with the use of wheelchair)
Companion Care
Companion Care is designed to monitor the personal well-being of individuals that require watchful oversight, keep them company and provide socialization opportunities. This includes, but is not limited to, assistance with bathing, mobility, meal planning and preparation, monitoring personal safety and dietary needs, providing medication reminders, incidental transportation, reading and letter writing, crafts, and escort to activities or to the doctors office.

We have caregiver professionals on staff and can accommodate your need for assistance, whether for a few hours per week up to daily care. Companion Care from Sweetwater Care is designed to meet your individual needs. Because our care is flexible, we are able to assist in a variety of ways. Whether you are in need of practical assistance with tasks, such as light housekeeping or transportation, or help with more personal tasks, such as bathing and dressing, we can provide the care you need. Respite
This service provides short-term care to relieve caregivers. Members who are cared for by Respite providers must be eligible for supports and services through the Division. Respite services are intended to temporarily relieve unpaid caregivers. Respite services are not intended as a permanent solution for placement or care.
Personal Care
Personal care is a non-medical service that is specifically intended to provide hands on support and/or supervision with activities of daily living. The level of Personal Care Services can vary widely depending on an individual’s needs, but the goal of this service is to play a vital role in maintaining a healthy and fulfilling lifestyle for individuals in need.

Personal care tasks may include, but are not limited to: Hands on or standby assistance with bathing, dressing, personal grooming, oral care, mobility, lifts/transfers, repositioning, toileting/incontinence care, nutrition/dietary care, range of motion activities, and dementia care.
Independent Living
Independent Living is a non-medical service offered to individuals who wish to live independently in their own home or apartment. Our care providers will work with the individual to foster independence and to assist and instruct them as they continue to learn and perform the daily tasks needed for an independent lifestyle.

Independent Living tasks we commonly provide as part of this service, include, but are not limited to: Assistance and instruction with personal health and hygiene, setting up medical and dental appointments, self-advocacy, maintaining a budget, menu planning, shopping and meal preparation, home maintenance and cleanliness, finding community resources, setting up public transportation, and setting up socialization opportunities.
Homemaker
Homemaker is a non-medical service providing practical assistance with regular household chores and related activities to enable an individual to remain in a safe, clean, and healthy home environment.
Homemaker duties include, but are not limited to the following: Meal planning and preparation, grocery shopping and errands, light housekeeping (weekly and daily tasks), laundry and linens, organizational tasks, taking out the trash, plant care and pet care.
